I had somebody say it to me once in an entirely genuine tone.

There was an asshole at work who pretty well constantly harassed me, finally he did something really nasty  and i made an actual complaint. I felt bad about causing a ruckus and I was apologizing to my boss and he says "for what? Finally standing up for yourself?  Good for you!"

I feel like I have to stress that I'm not being sarcastic when I saw that.

What I hate is "Bless your heart".  I just think "Well fuck you too".

I don’t think I can find a situation where it doesn’t sound like a petty dig. “Great to hear,” would make more sense for me to say if I’m not trying to take a shot at someone.

Its up there with... "thats crazy" for me.

Of im saying it... i don't really care.. if someone says it to me, I know they don't care

yeah, exactly....I've never said, "that's crazy" and meant it....it's just a plug and play response

if we all know what the game is, why even play?

first: that's a solid example of someone using it sincerely

second and more importantly: Yo, get ya boys and give him a wicked beatin' by the basketball courts

We don't trust anyone enough to be sincere.

If I am being sincere I would word it like "that's fucking bullshit!" assuming that's the same basic sentiment.

Because we'd rather not be told outright, "Stop talking, you're boring me."

Good for you usually gets a yes it is good for me response.

That's crazy is something I say to attempt to end a conversation because I don't fucking care.

Them - "My folks just died in a car accident."

Me - "That's crazy." (Logs off quickly).

Do they ever mean it?

Has that phrase always been a place holder for negative sentiment?

Yes, they do. Context and tone are your friends.
