Can someone please explain the appeal of Howie Mandel?

I liked him. He had an energy on stage that a lot of the comedians of his era all drove for but couldn't deliver on ....plus Bobby's World was the shit.  80/90s Howie is pretty boss.  But I assume you mean the current Howie. I don't see him often enough.

Though his face is very punchable.

Yeah, all of this. His face is still very punchable, plus he's bald now.

There are a few slappable or punchable faces in this world that I can name.

Howie Mandell does not come close to any of them. Most of the people I have in mind are also political figures.

See, most political figures I don't like, I go way beyond face punching and things get far more grim because their effect is far more damning that anything Howie can produce.

There are only 2 things I remember from "St. Elsewhere".  The ending episode, and Mandel as Dr. Fiscus performing his latex glove standup routine for someone.  That person doesn't laugh, and Mandel said something like, "Well, it was funny when the guy on tv did it".

I always wondered what brought on his "hide from the world, germaphobe" stage.

Thought about it some more & I think Mandel/Fiscus is in a hospital bed for some reason & 2 of the other doctors try to cheer him up by doing the latex glove routine, and Mandel doesn't laugh.
