The joke itself is conceptual, with the punch line changing with the times or, specifically, with the most recent social and sexual mores, but it's apparently been in continuous use since the beginning of the previous century. It's hallmark is the typical vaudevillian flourish with which the punchline and occasional follow-up are presented.
