I would suggest requesting an itemized statement and strongly state that if they are unable to provide such necessary information you are just going to have to presume that they are attempting to illegally double-bill you and you will react accordingly. You will either get the info you requested and it will help jog your memory [ including whether or not you already paid anything about that particular charge ] or they'll suddenly decide that maybe it's an error on their part and back off. 1

Even if there was something delinquent before you left the other job I don't understand how this would be the first you're hearing about it.... But it could just be since I have several visits a year for sarcoid, I watch my billing like a hawk. 1

I'd call the insurance company (after looking up the number, not calling one on the bill) and make sure it's legit and see wtf is going on
